Jenis dan Efek Complementary Therapy dalam Menurunkan Tekanan Intra Okular (TIO) pada Pasien Glaukoma: A Systematic Review

گلوکوم یکی از علل اصلی نابینایی دائمی در سراسر جهان است و پیش‌بینی می‌شود که تا سال ۲۰۴۰ به ۱۱۱.۸ میلیون نفر افزایش یابد. افزایش فشار داخل چشمی (IOP) یکی از علل اصلی گلوکوم است. درمان‌های کنونی برای کاهش IOP بیشتر شامل درمان‌های دارویی و مداخلات جراحی می‌باشد. با این حال، استفاده از درمان ضد گلوکوم دشوار بوده و نیازمند تعهد مادام‌العمر است و دارای عوارض جانبی درمانی می‌باشد. استفاده از درمان‌های مکمل ممکن است عوارض جانبی درمان، استرس و همچنین کاهش مفید IOP در بیماران گلوکوم را کاهش دهد. این مطالعه با هدف بررسی منابع مرتبط در مورد انواع و اثرات درمان‌های مکمل در کاهش فشار داخل چشمی در بیماران گلوکوم انجام شده است. این مطالعه یک مرور ادبیات است که از پایگاه‌های داده PubMed، ProQuest، Ebsco، Cochrane Library و Google Scholar استفاده کرده است.علاوه بر این، جستجوی ثانویه‌ای برای شناسایی انواع و اثرات درمان‌های مکمل در کاهش فشار داخل چشمی (IOP) بیماران مبتلا به گلوکوم انجام شد که در ده سال گذشته (2010-2020) منتشر شده، متن کامل، با طراحی RCT و به زبان انگلیسی بودند. نتایج: دوازده مطالعه مطابق با معیارهای ورود بودند و مورد بررسی قرار گرفتند. بر اساس نتایج این مرور، طبقه‌بندی درمان‌های مکمل استفاده شده شامل درمان انرژی (طب سوزنی، فشردن نقاط فشار و TENS) است که با کاهش فعالیت سیستم عصبی سمپاتیک در کاهش IOP مفید هستند، درمان ذهن-بدن-روح (موسیقی و مدیتیشن) که با کاهش کورتیزول و افزایش اندورفین‌ها مرتبط با کاهش IOP است، و درمان‌های مبتنی بر زیست‌شناسی (عصاره زعفران) به عنوان آنتی‌اکسیدان و نوروپروتکتور، که گونه‌های اکسیژن واکنشی را کاهش داده و مرگ سلولی مش شبکه‌ای ترابکولار را که نقش در کاهش IOP دارد، مهار می‌کنند. نتیجه‌گیری این مطالعه این است که درمان‌های مکمل می‌توانند گزینه‌ای در کنار درمان‌های متداول برای کاهش IOP در بیماران مبتلا به گلوکوم باشند.

Glaucoma is one of the main causes of permanent blindness worldwide and is predicted to increase in 2040 to 111.8 million people. Increased Intra Ocular Pressure (IOP) become one of the main causes of glaucoma. Current treatment for IOP more use of pharmacological therapy and surgical intervention. However, the use of anti-glaucoma therapy has been found to be difficult and requires a lifetime commitment and side effects of treatment. Using Complementary therapy may reduce the side effects of treatment, stress and beneficially lowering IOP in glaucoma patients. This study aims to review the relevant literature regarding the types and effects of complementary therapy at reducing intra-ocular pressure in glaucoma patients. This is a literature review study design using PubMed, ProQuest, Ensco, Chocrane Library and Google Scholar databases. Moreover, secondary searching conducted to identify types and effects of complementary therapy in lowering IOP of Glaucoma Patient, published on the last 10 years (2010-2020), full text, RCT design studies and in English. Result:Twelve studies fit into the inclusion criteria and were reviewed. Based on the results of the review, the complementary therapy classification used is Energy Therapy (acupuncture, acupressure and TENS) which are useful in lowering IOP by reducing the activity of the sympathetic nervous system, Mind-Body Spirit Therapy (music and meditation) by decreasing cortisol and increasing endorphins which correlate with decreasing IOP, Biological Based Therapies (saffron extract) as antioxidants and neuroprotectors, reduce reactive oxygen species and inhibit cell death of Trabecular Meshwork which plays a role in reducing IOP. The conclusion of this study, Complementary therapy may be an option in addition to conventional treatment in lowering IOP in glaucoma patients.
