زمینه: اثرات زعفران بر اختلالات خلقی قبلاً ثابت شده است. اخیراً، اثرات ضد توموری آن توجه زیادی را به خود جلب کرده است. این مطالعه با هدف بررسی اثرات مصرف کروکین در طول شیمی‌درمانی مبتنی بر داکسوروبیسین در سرطان پستان بر اضطراب، افسردگی و پروفایل سمیت شیمی‌درمانی انجام شد.روش‌ها: هفتاد و دو بیمار با سرطان پستان غیر متاستاتیک و مثبت برای Her2/neu یا سه‌گانه منفی، ثبت نام شدند و به طور تصادفی به دریافت ۳۰ میلی‌گرم در روز کروکین یا دارونما در طول شیمی‌درمانی اختصاص یافتند [۲:۲]. پرسشنامه‌های افسردگی و اضطراب بَک در ابتدا و پایان مطالعه مورد استفاده قرار گرفت. همچنین، معیارهای سمیت شایع ECOG برای ارزیابی عوارض جانبی شیمی‌درمانی اعمال شد.نتایج: ۳۶ بیمار در هر گروه ثبت نام شدند (جدول). پس از مداخله، میزان اضطراب و افسردگی در گروه کروکین به طور معنی‌داری کاهش یافت (p=0.001 برای هر دو) و در گروه دارونما به طور معنی‌داری افزایش یافت (p=0.006 و p=0.036 به ترتیب).لاکوپنی درجه II-IV به میزان قابل توجهی بالاتر بود (۴۷٫۲٪ در مقابل ۱۹٫۴٪، p=0.012) در گروه کروسین و واکنش حساسیت‌زدایی درجه II-IV (۳۰٫۶٪ در مقابل ۵٫۶٪، P=0.006) علاوه بر اختلالات عصبی (۶۶٫۷٪ در مقابل ۴۱٫۷٪، p=0.03) در گروه پلاسبو مشاهده شد. با یک پیگیری میانه ۱۶ ماهه، عود تومور در یک بیمار (۲٫۷٪) از گروه کروسین (متاستاز استخوان) و شش بیمار (۱۶٫۶٪) از گروه پلاسبو (عود موضعی: ۱ بیمار، متاستاز مغز: ۳ بیمار، متاستاز استخوان: ۲ بیمار) گزارش شد.

Background: The effects of saffron on mood disorders have already been established. More recently, its anti-neoplastic effects have provoked a great attention. This study aims to assess the effects of crocin administration during doxorubicin-based chemotherapy of breast cancer on anxiety, depression, and chemotherapy toxicity profile.
Methods: Seventy-two patients with non-metastatic Her2/neu positive or triple negative breast cancer were enrolled and randomly assigned to receive either 30 mg/ day of crocin or placebo during chemotherapy [2:2]. Beck’s Depression and Anxiety Inventories were used at baseline and end of the trial. Also, the ECOG Common Toxicity Criteria were applied to assess chemotherapy side-effects.
Results: 36 patients were enrolled in each group (Table). After the intervention, the degree of anxiety and depression decreased significantly in
the crocin group (p¼.001 for both) and increased significantly in the placebo-group (p¼.006 and p¼.036, respectively). There were significantly higher grade II-IV leukopenia (47.2% vs 19.4%, p¼.012) in the crocin group, and grade II-IV hypersensitivity-reaction (30.6% vs. 5.6%, P¼.006) in addition to neurological disorders (66.7% vs. 41.7%, p¼.03) in the placebo-group. With a median follow-up of 16 months, tumor recurrences were reported in one patient (2.7 %) of crocin (bone metastasis) and six patients (16.6 %) of placebo group (local recurrence: 1 patient, brain metastasis: 3 patients, bone metastasis: 2 patients).
Conclusions: The results indicate that using crocin during chemotherapy in patients with breast cancer has ameliorated anxiety and depression. Moreover, leucopenia increased whereas hypersensitivity reaction and neurological disorders decreased in the crocin group. Also, a trend towards survival improvement was observed which is going to be investigated on longer follow up.
